it looks like PHx conflict,
i have disabled PHx,
and put uncached call to Jot inside of chunk
than i made TV combo:

Allow comments on page=={{chunk_with_jot_using_placeholders}}||Disallow = Commenting is disabled for this page.

Same issue here although very different symptoms - seeing the form but not being able to post/moderate etc.

What a hair pulling 3 days - I'd even resorted to reading every post one-by-one and was about to start the entire website again with a completely new installation of Modx - OMFG etc etc

Unfortunately for me I think I need PHx but thank you for posting...
